[Sending-off / Rare Incident] A sending-off for two cautions: holding and delaying the restart of play

The holding offence, which deliberately stopped a promising attack (SPA), and the delaying of the restart that followed took place in one continuous sequence, but they are treated as two separate offences. Where the same player commits two such cautionable acts one after the other, it is correct to send the player off for receiving two cautions.
